- Two (2) Valid IDs (Company ID, Passport, Driver's License, SSS/GSIS, UMID, TIN, PRC, Postal ID)
- Proof of Billing (Electricity or Water)
- Proof of Income
- Business Owner: Record of Sales - Latest 3 Months
- Employed Individual: Latest 2 Months Payslips and Certificate of Employment with Compensation
